Midstate Mutual Insurance Company® is concerned with the privacy of its policyholders, customers, applicants for policies and claimants which may be referred to by the term "Customers and Consumers". This notice refers separately to Midstate Mutual by using the terms "us", "we" or "our", and it describes how we treat the information we receive.
Why We Collect and How We Use Information
We do collect certain nonpublic personal information called "Information" in the course of providing insurance coverage and in the handling of claims. We do not disclose Information about current or former Customers and Consumers to anyone except as authorized by you or by law. We use Information for business purposes including evaluating a request for our insurance or other products or services, evaluating claims, administering our products or services and processing transactions requested by Customers and Consumers. We may also use Information to offer Customers and Consumers other products or services we provide.
Kinds of Information Collected
We collect Information about Customers and Consumers from the following sources:
- Information we received from you on applications or other forms;
- Information obtained from inspections, surveys, on-site or telephone audits;
- Information about your transactions with us, our affiliates or others; and
- Information we receive from consumer reporting agencies.
If we need to verify Information or need additional Information, we may obtain Information from third parties such as adult family members, employers, other insurers, physicians, hospitals and other medical personnel. Information collected may relate to a Consumer and Customer’s finances, employment, health, avocations or their personal characteristics as well as transactions with us or with others including our affiliates.
How we Protect Information
We restrict access to Information about Customers and Consumers to those employees who need to know that Information to provide products or services to you. We maintain physical, electronic and procedural safeguards that comply with federal and state regulations to guard your information.
We may disclose any Information when we believe it is necessary for the conduct of our business, or where disclosure is required by law. For example, Information may be disclosed to others to enable them to provide business services for us, such as helping to evaluate requests for insurance, performing general administrative activities for us and assisting us in processing a transaction required by a Customer or Consumer. Information may also be disclosed for audit or research purposes; or to law enforcement and regulatory agencies, for example, to help us fight fraud. Information may be disclosed to affiliates as well as to others, such as companies that process data for us, companies that provide general administrative or legal services for us, other insurers and consumer reporting agencies. Our affiliates may include financial services companies such as property and casualty brokers, agents or companies, securities firms, financial advisors, affiliated banks, and may also include companies that are not financial services companies. We may make other disclosures of Information as permitted by law.
We may also provide Information to others outside Midstate Mutual such as marketing companies, to assist us in offering our products and services to Customers and Consumers, and financial services companies with which we have a joint marketing agreement, for example, an agreement with another insurer, agent or broker to enable us to offer certain of their products to Customers and Consumers. (See the Opt-Out Provision below.) We do not make disclosures of Information to other companies who may want to sell their products or services to Customers and Consumers. For example, we will not sell Information to a catalog company.
Customers and Consumers may request us to not disclose Information to affiliated third parties for such purposes as marketing other products or services. To tell us of your request, you may:

If you make this request, we will mark the record(s) for the(all) named insured(s) for the policy number you provide. Other parties to the policy including, but not limited to, mortgagees, lienholders, additional insureds, others to whom bills and cancellations are directed will not be marked.
